The attached Assumption of Risk form (Covid-19) and the consent/release form should be read carefully and printed out.  After printing a copy, please sign and date the second page (in both places).  This document is important and has to be signed and dated prior to your child being able to start conditioning during the Phase 1 of return to place.  Thank you!
